Overview
This article explains how to create a data capture form. Data capture forms are used to collect information from parents about their child when they enrol the child on a school trip. One or more forms can be attached to a school trip in Events.
Create a form
In Events, follow these steps to create a form that can be added to a trip:
- Select Forms.

From All Forms, select Create:

From Create Form:
- Enter a Title for the form.
- Enter a Description.

Add a question:
- Enter the question.
- Enter an answer to the question. This is used as one of the multiple-choice options for those completing the form.
- For each additional multiple-choice answer, select Add Answer, then repeat the last step (adding an answer).

After selecting all answers to the question, to add another question, select Add Question, and repeat the previous step (Step 4):

After adding all questions and corresponding answers, select Save.
You are returned to All Forms, where a message is displayed confirming the form has been save. The form is listed at the top of the tabular view:
Tips and best practices
- Forms can only be used with a single event.
- One or more forms can be added to a trip.

To delete a question from a form, select the delete icon:

To delete an answer to a question, select X:

You cannot change or delete a form used for an event after the event has been published (invitations have been sent out to parents).
