The Content Management System (CMS) lets you customise forms, surveys, and polls on your website to gather visitor information. When visitors submit their responses, they can receive an email copy, and you can also be notified of their answers by email. Responses may also be stored in a database, depending on how the system is configured. You can add forms to any page on your website, for example:
- A Get in Touch form could be added to your Contact Us page, or embedded into other pages too (see Juniper Websites: Embedding an existing Website Form).
- An application form could be added to the Admissions part of your website.
Internal forms you don't want to share with the public can be created for internal use. These can be stored in a hidden area (see Hiding an item in Sitemap View in Juniper Websites: Sitemap View). The view won't appear in the navigation menus. You can then use the component tool to embed it in other web pages, or share a direct link to parents.
Creating a form, survey or poll
If you have an existing form that you would like to copy, to create a new form, an administrator can duplicate it in Juniper Websites: Sitemap View. Edit the form with any changes after you have created it.
- Navigate to the area of the website where you want to create your form, survey or poll.
- Select the Form, Survey or Poll from the Create tab.
- Forms are used to gather information from your visitors, using a series of questions, for example, a Contact Us form, or trip authorisation form.
- Surveys are great for gathering data on the thoughts of others that can be analysed, for example, to understand their opinions on a given subject or their satisfaction levels. The results of a survey can be provided visually.
- Polls are useful to allow visitors to vote. Using this you can gauge opinions. The results of a poll can be output as a series of charts.
Add questions, and set up your form
To complete the set-up of your form, survey or poll Add Form Elements, then Update Form Settings.
